Use your Advantica Out-of-Network Vision Insurance Online

Follow the steps below to submit your claim:

Step 1

Download Claim Form

Full claim instructions may be provided on the claim form. Your policy or membership ID and/or authorization may be required. Service provider information can be found on the online receipt, which can be accessed by following Step 2.

Step 2

Print a copy of your online receipt.

Products or services must be paid for in full. Your insurance provider may require the HCPCS/CPT diagnostic codes for the listed products to be included on the Visionspecialists.org receipt. Each code can be handwritten next to the corresponding item on the itemized purchase receipt.

Claims may need to be filed within 365 days from purchase.
Advantica will only accept itemized receipts that indicate the services provided and the amount charged for each service.

How long does it take to receive reimbursement:

Many insurance providers will reimburse the purchase usually within 30-90 days. For details or status of a claim request, please contact the provider.
